Subject:           Revised Volunteer Program Forms

The OF-301 forms, used by the Bureau of Land Management (BLM) and other federal agencies to recruit and document volunteer service, have been revised. New versions are now approved by OMB for immediate use. The Volunteer Service Application – Natural and Cultural Resources (OF-301) (attachment 1), the Volunteer Service Agreement – Natural and Cultural Resources (OF-301a) (attachment 2) and the Volunteer Service Agreement – Natural and Cultural Resources Volunteer Sign-up Form for Groups (OF-301b) (attachment 3) were updated by the members of the Federal Interagency Team on Volunteerism (comprised of staff from the Bureau of Land Management, U.S. Fish and Wildlife Service, National Park Service, Bureau of Reclamation, U.S. Geological Survey, Bureau of Indian Affairs, U.S. Forest Service, National Oceanographic and Atmospheric Administration, and U.S. Army Corps of Engineers) and approved by the Office of Management and Budget (OMB).  For a complete overview of the revisions see OF-301 Forms: 2021 Revisions (attachment 4). The new forms are also available on the BLM’s Volunteer Program SharePoint site.

The new forms should be used starting immediately for documenting all new volunteers. Current volunteer coordinators may renew their existing volunteer service agreement form by renumbering it for the new year and having the volunteer and agency representative initial revisions, sign and date. As a reminder, all volunteer agreements must also have a Risk Management Worksheet (BLM Form 1112-5) (attachment 5). This form is also available on the BLM’s Safety, Health and Emergency Management SharePoint site.
